Indications of Splenectomy in Hematological Disorders

　血液疾患における脾臓摘出術の適応について解説した．脾摘術は治療目的と診断目的で行われる．前者の適応疾患としては，特発性血小板減少性紫斑病，遺伝性球状赤血球症，自己免疫性溶血性貧血などがあり，後者としては，悪性リンパ腫の病期診断，原因不明の脾腫などが挙げられる．それぞれの場合における注意点について，特に腹腔鏡下脾臓摘出術との関連で述べた．

Indications of splenectomy in hematological disorders are summarized. Splenectomy is indicated for two reasons ; therapeutic or diagnostic purpose. Major indications exist in idiopathic thrombocytopenic purpura, hereditary spherocytosis and autoimmune hemolytic anemia for therapeutic purposes, while in clinical staging in malignant lymphoma and splenomegaly unknown cause for diagnostic purposes.
