腹腔鏡補助下に手術を行った小腸平滑筋腫穿孔の1例

A Case of Perforated Leiomyoma Operated with Laparoscope

　小腸平滑筋腫穿孔に対して，腹腔鏡補助下に小切開開腹による手術が可能であったので報告した．患者は67歳，男性．穿孔性腹膜炎にて当院に紹介となった．胸部単純X線検査，腹部CT検査では腹腔内遊離ガスを認めた．緊急上部消化管内視鏡検索では胃，十二指腸病変を認めなかった．術前血行動態が安定していたため，発症から16時間後に小腸または大腸穿孔の疑いで腹腔鏡併用下に手術を施行した．小腸腫瘍の穿孔であったため病変直上で小切開開腹下に腫瘍切除術を行った．術後病理診断は小腸平滑筋腫であった．小腸穿孔に対しても，術中腹腔鏡は穿孔部位の同定に有用であり低侵襲な手術が可能と考えられた．

We performed a small-incision laparotomy under laparoscope on a perforated leiomyoma of the small intes-tine. The patient was a 67-year-old male who was referred to our hospital for perforative peritonitis. A scout roentgenogram of the chest and an abdominal CT scan revealed intra-abdominal free air. An emergency endoscopy of the upper gastrointestinal tract did not show any gastric nor duodenal lesions. Because the preop-erative hemodynamics was stable, we performed laparoscopic surgery at the 16th hour after the onset, suspect-ing perforation in either the small or large intestine.
