Strict regional lymph node dissection of gastric cancer, which requires skeletonization of adjacent impor-tant vessels and organs, is difficult to perform under laparoscope, because limitation of visual field, trouble in hemostasis, and difficulty in making a three dimentional operative field exist in laparoscopic surgery. Furthermore, blunt dissection is often used in laparoscopic lymph node dissection, which might increase the risk of cancer cell implantation.
