腹腔鏡補助下にHartmann手術後の再建を行った1例

A case of laparoscopically assisted colorectal anastomosis after Hartmann's procedure.

　腹腔鏡下手術の適応は大腸疾患にも広がっている．われわれは，汎発性腹膜炎を合併した虫垂周囲炎・S状結腸憩室炎に対してHartmann手術を行い，その後の腸管再建（人工肛門の閉鎖およびS状結腸・直腸吻合）を腹腔鏡補助下に行った1例を経験した．症例は58歳，男性．体位は砕石位とし，炭酸ガスで気腹した．トロッカーは下腹部に4本挿入した．直腸断端の同定と周囲組織からの剥離には，初回手術時に直腸断端に縫着しておいた長めのナイロン糸が有用であった．腸管吻合には吻合口径が25mmの自動吻合器（CDH 25：Ethicon社製）を使用した．Laparosonic Coagulation Shears（LCS）を用いることにより，良好な視野を確保しながら腸管を損傷せずに人工肛門部腹腔側のS状結腸周囲の脂肪組織の剥離が可能であった．術後の疼痛は軽く，22時間で排ガスを認めた．第4病日から経口摂取を始め，第17病日に退院した．手技は比較的容易で，今後の普及が予想される．

The indication for laparoscopic operations has been extended to colorectal diseases. We have experienced a case of periappendicitis and sigmoid colon diverticulitis with panperitonitis in a 58 year old male. The first operation was emergent Hartmann's procedure. The second operation was laparoscopy assisted reconstruc-tion of the gut, comprising closure of the artificial anus and sigmoidorectostomy. In the second operation, the patient was placed in lithotomy position and pneumoperitoneum was achieved using carbon dioxide gas. Four trocars were inserted into the abdominal cavity.
