腹腔鏡下胆嚢摘出術を行った胆嚢嚢胞の1例

A Case of Gallbladder Cyst that Underwent Laparoscopic Cholecystectomy

　胆嚢嚢胞は稀な疾患で，われわれが50歳，女性に対して腹腔鏡下胆嚢摘出術を行った径1.4cmのものは本邦で15例目と思われる．診断名が一定しないなどの問題があるが，世界的にみても数十例目と思われた．発生機序は様々であるが，後天性のものが大部分を占め，Rokitansky-Aschoff洞の粘膜面との交通が遮断されたと考えられるものが多い．われわれの症例では腺筋症は認められなかった．診断に当たっては，特徴的な症状はなく，術中超音波検査が有用であった．臨床的には鑑別診断が問題となる．文献的には癌の合併もあり，全身の精査が術前に必要である．胆嚢嚢胞は3cm以下のものがほとんどであり，腹腔鏡下胆嚢摘出術のよい適応と考えられた．

A cyst measuring 1.4 cm in the gallbladder body of a 50 year-old female, treated laparoscopically, is presented with a review of the literature. We found only 14 previous reported cases of gallbladder cyst (GBC) in Japan. We consider the entity sufficiently exceptional to warrant case report. Yet we could not decide how many previous cases existed in the world because GBC had various diagnostic names such as polyp, cystadenoma, cholecystitis glandularis, etc., and the pathology was often obscure. Only some tens of GBC cases seem to exist in the world medical literature.
