The concept of acute renal failure originated as an interpretation of a condition in which the renal function declines rapidly and has undergone various changes to arrive at the current diagnostic criteria. The classical approach to acute kidney injury is based on the site of onset, but adding the perspective of renal impairment in terms of the functional decline and structural (tissue) changes allows for a more detailed examination. In addition to systemic renal circulation, it is necessary to consider the microcirculation within the glomerulus in order to maintain perfusion for basic organ protection. This also requires an appropriate fluid volume assessment. Implementation of this basic approach is considered to be very important as a first step in the pathophysiological evaluation and treatment of acute kidney injury.
