6．カテーテル関連血流感染症予防—バンドルアプローチのその先へ

Prevention of catheter-related bloodstream infections

ICUにおいて，カテーテル関連血流感染症（CRBSI）が重要な医療関連感染症の1つであることに異論はないだろう。CRBSIの予防策に関して，「不要なカテーテルはすみやかに抜去する」などの原則は変わらないが，日々新たな予防策が登場し，ガイドラインの推奨も変化している。本稿では，ここ10年で新たに出てきた知見を中心にレビューし，最新のガイドラインなどにのっとり各種予防策を紹介する。

　現在は，バンドルアプローチとして複数の予防策を組み合わせる方法が主流である。しかし，新たなエビデンスを伴った予防策が続々と登場するなかで，今後はそれら予防策の取捨選択が求められてくるかもしれない。

There is no doubt that catheter-related bloodstream infections are one of the most important iatrogenic infections in the intensive care unit. Although principles such as “remove nonessential catheters” remain unchanged as preventive measures for catheter-related bloodstream infections, new preventive measures are constantly emerging and guideline recommendations are changing. In this article, new findings in the past 10 years are reviewed and various preventive measures in accordance with the latest guidelines are introduced. Currently, the bundle approach, combining multiple preventive measures against catheter-associated bloodstream infections, is commonly used. However, as new evidence-based preventive measures continue to emerge, it may be necessary in the future to select and choose among the available measures.
