7．COPD患者—予後予測，疾患経過，ACPの進め方

The prognosis of patients with chronic obstructive pulmonary disease

慢性閉塞性肺疾患（COPD）は，今日でも国内外で重要な死亡原因の1つであるが，その終末期においては十分なケアが行われているとは言い難い。その理由として，主に増悪と寛解を繰り返しながら終末期に向かっていくという慢性疾患としての特徴により，終末期を認識しにくいことが挙げられる。本稿では，これまでに報告されているCOPDの短期および長期予後に関する予測ツールについて，さらに，気管切開術を受けた患者の生活，予後についても述べる。具体的にどのようなCOPD患者で終末期を意識して，疾患軌道に合わせて，どのようにアドバンス・ケア・プランニング（ACP）を進めていくかについても解説する。

COPD （Chronic Obstructive Pulmonary Disease） is leading causes of death in Japan and other countries, but not enough care is given to patients at the end of life. The reason for insufficient care is the difficulty recognizing the end-stage of the disease, which is characterized as a chronic disease that progresses toward the end of life with repeated exacerbations and remissions. This review describes the predictive tools for short- and long-term prognosis of patients with COPD that have been reported, and discusses the prognosis of patients who have undergone tracheostomy. It also discusses how ACP （Advance Care Planning） can be implemented in any patient with COPD at the end of life and in accordance with the disease trajectory.
