COPD (Chronic Obstructive Pulmonary Disease) is leading causes of death in Japan and other countries, but not enough care is given to patients at the end of life. The reason for insufficient care is the difficulty recognizing the end-stage of the disease, which is characterized as a chronic disease that progresses toward the end of life with repeated exacerbations and remissions. This review describes the predictive tools for short- and long-term prognosis of patients with COPD that have been reported, and discusses the prognosis of patients who have undergone tracheostomy. It also discusses how ACP (Advance Care Planning) can be implemented in any patient with COPD at the end of life and in accordance with the disease trajectory.
