近年,敗血症の発症率の上昇や院内死亡率低下の結果,敗血症生存者sepsis survivorsが増加傾向にある。一方で長期予後(長期死亡率,再入院率,身体および認知機能など)が悪いことが問題視されている。sepsis survivorsの長期予後を改善させた介入は,リハビリテーションの継続,入院中のABCDEFGHバンドルのみである。我々医療者にとってsepsis survivorsの長期予後を改善することは優先課題であり,現在有用と考えられている介入の確実な実践と,さらなる知見の探索,および予後の改善が見込めない患者への適切な対応が求められる。
In recent years, the number of patients who survive sepsis has been increasing due to an increasing incidence and decreasing in-hospital mortality. However, there are no improvements in long-term prognosis (long-term mortality, rehospitalization, physical and cognitive function, etc.). There are no reports of improved long-term outcomes in patients who survive sepsis other than continuation of rehabilitation and ABCDEFGH bundles in the hospital. Improving the long-term prognosis of patients who survive sepsis is a priority for us as health care providers, and we must ensure that the interventions we currently consider useful are implemented, search for new approaches and respond appropriately to patients whose prognosis does not improve.
