Postoperative nausea and vomiting (PONV) is a serious and frequent complication in patients undergoing surgery performed with general anesthesia. Approximately 30% of all patients experience PONV, and in a subpopulation of high-risk patients, the incidence increases to 80%. This uncomfortable experience decreases patient satisfaction and causes poor-quality anesthesia and intensive care. Previous guidelines and consensus statements have addressed recommendations for identifying high-risk patients, managing baseline PONV risks, choices for prophylaxis, and rescue treatment of PONV. In 2020, the newest guideline was established for surgeons and anesthesiologists in the United States. Here, we summarize and discuss the current clinical landscape of PONV based on the novel guideline and recent publications for intensivists.
