Postoperative transfer from the anesthesiologist to the intensivist can be performed appropriately when the three elements of “information,” “responsibility,” and “environment” are harmonized in a way that is commensurate with the effort. Specifically, it is most important to standardize “information” by using checklists and templates, followed by coordinating the communication and tasks of medical personnel to ensure the quality and quantity of “responsibility”. In addition, it is also effective to prepare the “environment” by using standardized methods and implementing changes in the awareness of medical personnel regarding the transfer. Since the “environment” in each facility differs greatly, it is important to optimize the transfer of information through the “plan-do-study-act” (PDSA) cycle.
