Epidural analgesia can provide optimal pain relief after major surgical procedures. Epidural anesthesia has various physiologic effects on cardiovascular and respiratory function. Early meta-analyses have shown that the use of epidural anesthesia and analgesia is associated with good perioperative analgesia and other benefits including reduced postoperative morbidity. Recently, these benefits have become less apparent because of the increasing use of minimally invasive surgery and changes in postoperative management. Severe neurological complications occur more frequently than previously believed. Thus, epidural analgesia may not be a part of a standard care for most patients undergoing surgery. However, epidural analgesia is still the first choice for high-risk patients undergoing major open surgery. Intensivists should understand the indications, placement techniques, solutions administered, and potential complications for epidural analgesia for pain management in the ICU.
