This article reviews the physiology of oxygen delivery and consumption, the pathophysiology of shock, and parameters for the evaluation of oxygen supply-demand balance, all of which are fundamental knowledge for critical care physicians. We summarize the oxygen cascade from the atmosphere to supplying oxygen to tissues. We then discuss the physiology of oxygen delivery (DO2), oxygen consumption (VO2), and the oxygen extraction ratio (O2ER). Parameters for the evaluation of oxygen supply-demand balance are elucidated, including mixed venous oxygen saturation (SvO2), central venous oxygen saturation (ScvO2), serum lactate level, mixed venous-to-arterial carbon dioxide tension difference (P(v-a)CO2), central venous-to-arterial carbon dioxide tension difference (P(cv-a)CO2), and oxygen reserve index (ORI).
