Despite recent contradictory data, there is a consensus that very early rehabilitation (within 24 hours) of the patient after an acute stroke reduces mortality and improves functional outcomes. However, important questions regarding adequate frequency and intensity of mobilization after acute stroke remain to be investigated. While the effects of early physiotherapy on cerebral perfusion and intracranial pressure have not been fully studied in these patients, a recent report shows that very early mobilization of patients with acute ischemic stoke treated with recombinant tissue plasminogen activator may lead to favorable functional outcomes without adverse effects. Swallowing abnormalities after acute stroke also need to be discussed, and may require interventions such as early screening and swallowing training. The detection of cognitive impairments and associated disabilities in patients after an acute stroke is another critical issue to discuss. As a promising technique to facilitate efficient early mobilization, a novel robot-assisted rehabilitation program deserves introduction to the readers of Intensivist.
