4) 強皮症

Systemic sclerosis

全身性強皮症は，皮膚の硬化だけでなく，血管病変の合併がほとんどの症例でみられる．その2種類の病態に同時に対応できる治療薬は今のところない．線維化に対しては，免疫抑制薬での治療となり，血管病変に関しては血管拡張薬での治療となる．この10年で，治療薬の有効性がかなり明確になってきた．線維化病変には，シクロホスファミドや他の免疫抑制薬，血管病変には，プロスタノイド，エンドセリン受容体拮抗薬，ホスホジエステラーゼ5阻害薬を用いることが多くなってきている．しかし，まだ治療に反応しない症例は多く，近い将来の病態解明と新規治療薬の開発が待たれる．

Systemic sclerosis is a connective tissue disease characterized by tissue fibrosis and endothelial injury.　In the last decade, several treatments for fibrosis and endothelial damage have been recommended from Europe, USA, and Japan.　Fibrosis is treated by immunosuppressants including cyclophosphamide, and endothelial damage is treated by several kinds of vasodilators.　Those therapies are effective and safe.　Especially, endothelin receptor antagonist and phosphodiesterase 5 inhibitor are efficacious for the treatment of pulmonary hypertension and digital ulcers.
