Regulation of the pancreatic β-cell mass involves a balance of replication, apoptosis, and regeneration. The mechanism underlying decreased β-cell mass in type2 diabetes is increased apoptosis. However, apoptosis is a difficult process to measure because the dead cells are rapidly degraded. Therefore, studying remaining beta cell mass replaces quantification of apoptosis. Acute insulin response after glucose load appears to better predict beta-cell mass than fasting measures, such as the HOMA-β. Imaging modalities such as MRI and PET could be used clinically in the near future as methodologies for non-invasive imaging of islet mass. OPT would be more suitable for monitoring islet mass and allow whole pancreas 3D spatial analysis.
