副鼻腔に浸潤した多発性頭蓋内髄膜腫の1例

Multiple Intracranial Meningiomas Invading the Paranasal Sinuses: Report of a Case

I．はじめに

　副鼻腔の髄膜腫の報告はきわめて稀である。その発症様式によって頭蓋内より進展するものと副鼻腔に異所性に発生するものの2群に分けられる。一般的に髄膜腫は徐々に増大する良性腫瘍であり加えて手術により完全治癒が期待できるので，頭蓋内より進展した副鼻腔髄膜腫に耳鼻咽喉科医が遭遇するのは珍しい。

　最近筆者らは髄膜腫が中頭蓋底および前頭蓋底に多発性に発生し篩骨篩板より左篩骨洞および左固有鼻腔に波及した1例を経験したので報告する。

　A very rare case of multiple intracranial meningiomas invading the paranasal sinuses with hyperostosis of the skull is presented.

　The patient is a 13-year-old female with the complaint of the nasal obstruction for 6 months prior to admission.

　A hard mass occupied in the left nasal cavity. We underwent a partial resection for the purpose of improvement of the nasal obstruction. Pathological examination revealed meningotheliomatous meningioma.

　The present patient is the ninth case reported in the world literature during the last forty years.
