耳下腺部に発生した線維性組織球腫の1症例—免疫・病理組織学的観察

Fibrous Histiocytoma of the Parotid Gland: A Case Report

I．はじめに

線維性組織球腫（fibrous histiocytoma）は軟部組織に多く発生する腫瘍で，臨床的に再発率が高くまた近年の免疫組織化学の進歩に伴い腫瘍細胞鑑別とその組織発生について非常に興味のもたれる腫瘍の一つである。頭頸部領域でも鼻・副鼻腔や喉頭などで多くの報告をみるが，耳下腺部のそれは非常に珍しく過去に6例の報告をみるのみである（表1）^1〜6）。今回右耳下腺部に発生した線維性組織球腫の1例を経験したので，その臨床的特徴を文献的考察を加え報告するとともに免疫組織化学的，電顕的に腫瘍組織を観察しその組織発生についても検討した。

　A 47-year-old woman with fibrous histiocytoma in the right parotid gland was reported. This tumor is extremely rare in this region and six cases have been reported. The clinical features of this case with literature were reviewed and tumor tissues were examined with immunohistochemical and electron microscopic techniques.

　The histogenesis of this tumor is controversial, however, our electron microscopic findings supported an origin from mesenchymal cells which differentiate along a broad fibrohistiocytic spectrum.
