人凍結乾燥硬膜によるFrey症候群予防法の遠隔成績

Prevention of Frey's Syndrome Using Lyophilized Human Dura: A Following-up Study

I．はじめに

　Frey症候群（味覚性発汗症）は耳下腺手術後に高率に発症する不快な合併症の一つでしかもいったん発症するときわめて難治である。著者らは本症候群の発症を抑えるべく耳下腺手術時に人凍結乾燥硬膜を挿入する方法を考案し，その有用性を本誌に予報的に報告^1）した。しかし本法に同症候群の発症を予防する効果が永続的にあるや否やを論ずるには長期にわたって追跡調査が必要である。

　今回人凍結乾燥硬膜挿入術後の遠隔成績を検討したところ本術式はFrey症候群の発症を抑止するに優れた方法であることが再確認されたので，長期観察例の概要を報告する。

　The authors devised a new surgical procedure to prevent Frey's syndrome by the insertion of a lyophilized human dura as a barrier to the regenerating fibers of the auriculotemporal nerve. In this paper, the effectiveness of the above procedure was studied in 13 patients with a post-operative observation period from one and half to five years. Only one of these patients was clinically aware of Frey's syndrome at the fifteenth month after the operation. The other 12 patients had excellent long-term results. Therefore, the insertion of a lyophilized human dura seems to be a valuable method to prevent Frey's syndrome.
