耳下腺acinic cell tumorの2例

Acinic Cell Tumor of the Parotid Gland: Report of 2 cases

　I．緒言

　耳下腺腫瘍はきわめて多彩な病理組織像をもち，それ故に諸家によって種々の分類がされている。耳下腺のaciner-like-cellからなる腫瘍は古くはadenomaと分類されていたが，その悪性症例の存在から，acinic cell adenocarcinoma^1〜3），あるいはacinic cell carcinoma^4〜8）と分類され，一方，良性所見を示すこともあるため，acinic cell tumor^9〜11）と分類されることもある。最近では，1972年WHOの分類^12）以来，acinic cell tumorの呼称が一般化されているようである。

　この腫瘍の報告は，欧米では比較的多いが，本邦では少ない。最近，筆者らは臨床的にも興味ある経過をとったacinic cell tumor 2例を経験したので，報告すると同時に，その病理学的検討，および筆者らが常々述べている術前総合診断法の有用性についても述べる。

　Two cases of acinic cell tumor of the parotid gland were reported. In the first case a tumor was as big as a child's head, and its histopathological examination showed a well differenciated type and "solid pattern" of growth.

　In the second case the tumor was metastasized in the neck, and its histopathological examination showed a low-differentiated type and "microcystic pattern" of growth. Preoperative diagnosis was effectively established by using CT, RI and ultrasonics in both cases.
