鼻性視神経炎の2症例について

Rhinogenous Optic Neuritis: Report of Two Cases

I．はじめに

　日常耳鼻咽喉科診療にさいしておそらくいままで鼻性視神経炎に気づかずに副鼻腔炎の治療を行っていた症例があったのではないかと思われる。眼科より球後視神経炎と診断され，副鼻腔炎の手術により鼻性視神経炎であることを確認しえた症例，ならびに急性副鼻腔炎で症状経過より後部篩骨洞炎による鼻性視神経炎であった症例を経験したので，若干の文献的考察を加えて報告する。

　The author reported two cases of rhinogenous optic neuritis.

　Case 1 : A 68-year-old male who had chronic inflammation of the left nasal accessory sinuses. He was suffering from frequent disorders of visual acuity and abnormal visual field. Inflammation of the posterior ethmoid sinus and abnormal eminentia optica were detected at time of operation.

　Case 2 : A 59-year-old female had acute inflammation of the nasal sinuses. Despite the findings of a marked choked papilla in the fundus oculi, the patient had a very slight subjective symptom. Both x-ray and computed tomography examinations showed a clear shadow in the right posterior ethmoid sinus. This patient received the conservative therapy.

　Both patients had no recurrence for more than 16 months.
