喉頭癌の治療成績の検討

Study on Treatment for Laryngeal Carcinoma

I．はじめに

　喉頭癌は頭頸部悪性腫瘍の中では比較的予後のよいものとされており，治療法もほとんど確立されているかにみえる。しかし症例によっては未解決の問題も多い。1）Stage Iで放射線単独で根治できない放射線抵抗性癌があること^1〜4）。2）Stage IVではN，とくにN₃が予後を大きく左右^5,6）し，手術と照射のみでは治療成績に限界がある^7）ことなどである。これらの問題点について臨床病理組織学的に検討し，照射と手術の限界について若干の知見を得たので報告する。

　Treatments of 69 previously untreated patients with squamous cell carcinoma of the larynx seen from 1976 to 1982 were reviewed. Patients were grouped by stage (UICC) and modality of treatments.

　Radiation alone or surgery alone or combined therapy was carried out. The actuarial 5-year survivals were Stage I 93 %, Stage II 86%, Stage III 58% and Stage IV 13%.

　Clinicopathological factors affecting local failure of irradiated tumor were discussed. Our data suggest that postoperative radiotherapy decreases tumor recurrence in patients without extracapsular tumor spread.
