 A 37-year-old man with uncontrolled diabetes mellitus complained of visual disturbance and IIIrd, VIth cranial nerve paralysis. Radiological examination showed bone destruction of the antrum and hard palate. Malignant tumor of the maxillary sinus was suspected, but exploratory antrotomy revealed mucormycosis. Treatments included surgical debridement, Flucytosine for mucor and insulin for diabetes mellitus. The patient showed a good postoperative course.

 The cause of the spotty destruction presented radiologically was estimated as follows: Mucor invaded blood vessels resulting in thrombosis and tissue infarction. Then the antral bone around the penetrating vesseles was early destructed by the mucor, but the other part of the bone might remain without destruction, hence the remnant of bone showed spotty.
