顔面にみられた"いわゆるhairy polyp"の1例

A Case of "So-called Hairy Polyp" on the Face

I．はじめに

　鼻部に発生する先天性腫瘤はきわめて稀であり頭蓋底と鼻腔の発生学上の異常と考えられる。Macomberら^1）は先天性鼻副鼻腔腫瘤を発生学上の差異により，神経原性腫瘤である鼻神経膠腫や鼻脳ヘルニアと外胚葉性腫瘤である類皮腫，中胚葉性腫瘤である血管腫や母斑とに分類している。一方結合組織由来の真性腫瘍である奇形腫（広義は大きく四つに分類され，類皮腫，奇形腫様腫瘍，奇形腫（狭義），上顎体とに分けられる^2）。

　今回われわれは外胚葉性成分と中胚葉性成分をもつ奇形腫を鼻前庭から鼻腔にかけて認めた1例を経験したのでここに報告する。

　A rare case of "so-called hairy polyp" in a 15-day-infant is reported. The patient had a mass at the upper lip after birth. The mass, 4×4.5×3.5 cm in size, originated from the left nasal vestibule and was covered by skin. The mass hung over the mouth, but the mental and physical developments were not disturbed. The mass was removed surgically one year later under general anesthesia. Skin defect, 3×2.5cm in size, was covered by full thickness skin from the thigh. Pathological study of this mass revealed many types of different tissues. Their components were ectodermal and mesodermal. It was histopathologically diagnosed as "so-called hairy polyp", one of teratomas.
