小脳橋角症候群を呈したV-Zウイルス感染症

Cerebellopontine Angle Syndrome due to Varicella Zoster Infection: Report of a Case

I．はじめに

　小脳橋角症候群を呈する病変として，聴神経腫瘍，髄膜腫を主とした腫瘍性病変のほかに，非腫瘍性病変としては，クモ膜嚢腫，クモ膜癒着の報告がみられる^1）。また，めまい，突発性難聴，顔面神経麻痺を同時期にきたす疾患としては，Hunt症候群が知られているが，その経過を追跡した詳細な報告は比較的少ない。今回，私どもは，突発性難聴で発症し，小脳橋角症候群の所見を呈したが，血清および髄液中のV-Zウイルス抗体価に有意な変動を示し，ヘルペス所見を欠いたHunt症候群の亜型とも考えられる症例を経験したので，若干の考察を加え，その臨床経過の大要について報告する。

　Cerebellopontine angle syndrome after varicella-zoster infection in a 72-year-old woman was described.

　She was admitted to hospital with polycranial neuropathy. She caught cold and headache one week previously. Six days after she noticed sudden deafness and facial palsy next day. Air CT cisternography was performed, but acoustic tumor was not found. The varicella-zoster antibody titer in serum and cerebrospinal fluid was elevated significantly, but no herpes eruption was found on her body. We considered this case to be an atypical Hunt syndrome, and emphasized the necessity for virological investigation for polycranial neuropathy.
