Tinnitus of Muscular Origin

I．はじめに

　筋肉を音源とする耳鳴は耳管筋性と耳小骨筋性に大別される。前者の発生部位や機構などについては，熊沢^1）や白幡^2），成瀬^3）らの詳しい報告があるから，著者は自覚的耳鳴との関連性という観点などに立って，若干の考察を試みるにとどめ，主に耳小骨筋性耳鳴について述べる。

　耳小骨筋には横紋筋である鼓膜張筋とアブミ骨筋とがあり，前者は三叉神経第三枝，後者は顔面神経の支配下にあるが，不随意筋である。しかしきわめて稀に耳小骨筋を随意的に収縮できる人があり，このさいに耳内に生ずる雑音が耳鳴として自覚されることがある。たとえば角^4）は自分が耳小骨筋を随意的に収縮させうることを見出し，そのさいにかなり低音の痙攣様筋収縮音を自覚し，最大10数秒まで持続させている。どちらの筋肉かは未確認であるが，鼓膜張筋であろうと推察されている。

　Contraction of the intra-tympanic muscles produce tinnitus, although occurrence rate of the tinnitus is very low. The author described the out-lines of next points.

　3) differential diagnosis between the stapedius muscle tinnitus and the tensor tympani muscle tinnitus

　4) therapy of tinnitus

　Regarding tinnitus due to contraction of the tubal muscle, the author considered the following items. 1) objective or non-objective ?, 2) timbre, 3) continuous or intermittent ?, voluntary or involuntary ?, 4) bilateral or unilateral ?, 5) tubal catheterization as the diagnostic method and 6) objective record.
