急激な両耳の内耳障害をきたした陰蔽性乳様突起炎（masked mastoiditis）の2症例

Two Cases of Sudden Progressive Cochlear Deafness Induced by Masked Mastoiditis of Bilateral Ears

I．はじめに

　急性中耳炎は抗生物質により容易に治療されるようになったが，一方では，不適切または不徹底な抗生物質投与により，鼓膜の穿孔，大量の耳漏，外耳道後壁の腫脹などの典型的な急性乳様突起炎の臨床症状は示さないが，乳突蜂巣に炎症病巣が遺残し，これが難聴や頭痛などの原因となっている陰蔽性乳様突起炎が少なからず見られるようになった。筆者らは，急激に進行する両側の内耳障害をきたした陰蔽性肉芽性乳様突起炎の2症例を経験した。両症例とも手術的治療を行ったが，内耳機能を回復し得たのは1例で，他の1例では1耳は聾，1耳は高度感音難聴となり，日常会話は不可能となった。2症例共，実地臨床医家にとって留意すべき問題のある症例と考え，ここに報告し，御批判を仰ぎたい。

　The clinical features of the two cases were as the following; 1. bilateral lesions, 2. lack of the typical symptoms of acute otitis media such as otorrhea and perforation of the ear drum, 3. bilateral sudden progressive cochlear hearing loss, and 4. the existence of masked mastoiditis with granulation. The etiology and therapy of masked mastoiditis were shortly discussed, and the authors emphasized that urgent surgical treatment was necessary for masked mastoiditis which showed delay of improvement and/or progressive sensorineural hearing loss.
