Perilymph Gusher: A Case Report

I．はじめに

　perilymph gusher（stapes gusher）は，アブミ骨手術の折に，アブミ骨底板を開窓した際に，多量の外リンパが湧出する状態をいい，クモ膜下腔と，内耳外リンパ腔との問に交通に異常が存在することが想像される疾患である。本疾患は，様々な奇形を合併することが多いが，我々は粘膜下口蓋裂を伴ったvan der Hoeve症候群の患者にアブミ骨手術を行い，perilymph gusherとなった症例を経験したので報告し，併せて文献的考察を加えた。

　Perilymph gusher, a rare complication of stapedectomy, in van der Hoeve syndrome was reported and discussed.

　The patient had a moderate hearing loss of mixed type but no vertigo and meningitis. At surgery, moderate anomalies of the ossicles were found, and massive perilymph leakage occured when the footplate was removed.
