両側内耳道の著明拡大を伴ったperilymph gusherの1症例

Perilymph Gusher with Profound Bilateral Internal Auditory Canal Enlargement

I．はじめに

　perilymph gusherはアブミ骨手術のさいにみられる異常であり，クモ膜下腔と内耳リンパ腔の間に広い交通路が存在するためにアブミ骨底板の開窓とともに多量の外リンパが勢いよく流出する状態をいうが，術前にその発生を予測することは困難と考えられている。今回われわれは両側内耳道の著明拡大を伴う家族性難聴で，鼓室試験開放のさいアブミ骨へのわずかな操作でperilymphgusherに見舞われた症例を経験した。両側内耳道拡大に両側難聴を伴う例はきわめて稀であり，珍しい症例として報告するとともに若干の文献的考察を加えてみた。

A 19-year-old man with bilateral hearing loss of a mixed type was presented with roentogeno-grams of Stenvers' view and CT scan showing an extreme dilatation of the internal auditory canal, but no tumor. An exploratory tympanotomy of the left ear was performed. An abrupt perilymph gush occurred when the stapes was touched slightly.

The routes of cerebrospinal fluid gush were considered both the internal auditory canal and the cochlear aqueduct. The relation between the hearing impairment and the enlarged internal auditory canal was discussed.
