耳下腺唾石症例—ならびに本邦におけるその統計的観察

Case Report of Parotid Sialolithiasis

　I．緒言

　唾液腺内に生じる結石，すなわち唾石症は日常臨床で時に遭遇する疾患であって，かならずしも稀有なものでない。しかしこの唾石は顎下腺や舌下腺に発症することが多く，耳下腺の唾石は少ない。全唾石症に占める耳下腺唾石は欧米では6〜10％といわれている（Rauchら^34）Ivyら^35））。一方，本邦の統計によると，その頻度は5〜10％（久保ら^12），田縁ら^23），五味^36），小林ら^37），村上^38））。であり，発症は少ないようである。筆者らは最近，19歳の女性の耳下腺唾石を経験したので，その症例を供覧するとともに，筆者らが渉猟し得た本邦の耳下腺唾石の報告例について統計的観察を行ったので，若干の考察を加えて報告する。

　Parotid sialolithiasis of a 19-year-old girl was reported. The examinations showed purulent discharge from the parotid duct orifice and a palpable small hard resistance around the left parotid duct. The surgical operation was performed with a small incision in longitudinal direction along the parotid duct. A stone, ricegrain in size, was removed and examined under the scanning electron microscopy, and was analysed by x-ray microanalyzer.

　Since the incidence of sialolithiasis in the parotid gland is usually lower than that of the submandibular gland, the statistical observations of parotid sialolithiasis reported in our country were made and the results were discussed briefly.
