CNVの臨床応用—特に心因性難聴に対する診断的意義

Clinical Use of CNV in Objective Audiometry: Result and Evaluation for Psychogenic deafness

　I．はじめに

　CNV（contingent negativc variation）は，Walterら^1）が，予告となる第1刺激と命令刺激となる第2刺激との間に生ずる陰性の電位変動を発見し命名したもので，別名cxpectancy waveとも称されるように，第1刺激を認識した結果，次の第2刺激の出現を期待すると生ずるものとされている。

　すなわち，この電気現象には"認識過程"そのものが客観的に表出されるため，これを聴力検査に応用した場合の臨床的意義はきわめて大きい。すでにBrianら^2,3）が語音聴力検査に，Prevecら^4）が純音聴力検査に応用し，また，本邦では佐藤ら^5）がCNVの臨床応用に対する基礎的検討を行っている。CNVはその特徴から詐病ないし心因性難聴の聴力検査には，特に有効な検査手段になると思われる。筆者らは，これらを疑う症例に同検査を施行し，陽性所見を得たので報告する。

　Contingent negative variation (CNV) was clinically used in objective audiometry in normal hearing subjects and subjects with functional deafness.

　CNV was recorded in most of the patients suspected of psychogenic deafness. This fact showed that psychogenic deafness differed from a deafness that has been termed as "disturbance of sound perception" or "sleeping condition of hearing activity.

　These findings may produce complicated problems about functional deafness. The differential diagnosis between so-called "psychogenic deafness" and "malingering" became rather difficult according to the results of our study.
