糖尿病による突発難聴が疑われた小児心因性難聴の1例について

A Case of Psychogenic Deafness Suspected of Sudden Deafness due to Diabetes Mellitus

はじめに

　最近，難聴を訴えて来院する小児の中で，いわゆる機能性難聴ないし心因性難聴の症例が稀でなくなり，その傾向は年々増加しているように思われる^1〜8）。したがって，これを少しでも疑う患児については一般聴力検査に加えて他覚的聴力検査も施行し，難聴の有無を確認する必要があるが，時として治療経過中に初めて本症と診断され，反省させられる難しい症例もある。今回，われわれは糖尿病による突発難聴と診断して治療を始め，その経過中の検査所見からようやく診断のついた心因性難聴の小児の1例を経験したので反省も含めて報告する。

A 13-year-old girl having glycosuria presented with a sudden onset of hearing loss with tinnitus in her right ear. Pure tone audiometry revealed a sensorineural deaf and Bekesy test showed Jerger II type. The hearing deteriorated gradually in spite of the intensive treatments. However, an ABR test showed no differences between the right and the left ear, and she made a complete reco-very by psychotherapy.

The correct diagnosis was difficult, because no controversial signs were found except ABR test. The authors stress the necessity of a careful follow-up and an appropriate test to diagnose the psychogenic deafness.
