螢光色素Fluorescein sodiumによる鼻粘膜手血管透過性測定の試み

FLUORESCEIN LEAKAGE MEASUREMENT FOR CAPILLARY PERMEABILITY OF HUMAN NASAL MUCOSA

　Ⅰ．はじめに

　鼻過敏症者はいろいろな内的あるいは外的刺激に対して鼻粘膜の感受性が高く，その結果，種々の病態生理的変化が鼻腔内に生じ，くしやみ，水様性鼻漏，鼻閉塞などの症状が容易に誘発される。鼻過敏症の診断に関しては各方面からの検討がなされている。これらの病態生理的変化を検索することにより鼻過敏症を同定しようとする試みも少なからずなされ今日では臨床検査法として応用されているものもある^1）。

　今回われわれは鼻過敏症を同定する1つのfactorとして鼻粘膜毛細血管の透過件に着目しfluorescein sodium（以下fluo. と略す）を用いてその透過程度の定量的測定を試みた。生体において鼻粘膜毛細血管の透過性を定量的に測定する方法は従来報告されていない。本法は新しい試みであり，かつ，その測定の結果，鼻過敏症例と鼻正常例との間に差異を認めたので若干の考察を加えて報告する。

　Measurement for capillary permeability of human nasal mucosa was studied by using intravenous injection of fluorescein in 11 healthy subjects, in six with allergic rhinitis and in one who received vidian neurectomy.

　The method applied was as follows ; small pieces of filter paper were placed on the inferior turbinates and the septum, and were removedfor measurement at 30 minutes after i.v. administration of 10 ml of 10% fluorescein sodium. The filter papers were put in 1. 0ml of phosphate buffer at pH 7.4, and kept cool and dark place for over 2 hours. Fluorescein concentration in this solution was measured with slitlamp microphotometer.

　As the results, fluorescein leakage from the nasal capillary was more significantly in the subjects with allergic rhinitis than in the healthy subjects. This is the first paper reporting fluorescein leakage measurement for capillary permeability of human nasal mucosa.
