副鼻腔に原発した悪性線維性組織球腫の一剖検例

A MALIGNANT FIBROUS HISTIOCYTOMA OF THE MAXILLARY SINUS

　I．緒言

　線維性組織球腫fibrous histiocytomaは組織球由来の腫瘍で，四肢，体幹，後腹膜腔，頭頸部の軟部組織に好発することはすでによく知られている^1）。この腫瘍は良性のものが多いが，局所再発率は高いものの全身的転移など臨床的に悪性縁を示すものは比較的少ないとされている^2）〜4）。しかも鼻腔副鼻腔に原発するものはきわめて稀である^5）〜9）。

　今回われわれは，左上顎洞，篩骨洞にかけて原発し，全身へ血行性に転移を示した悪性線維性組織球腫malignant fibrous histiocytomaの1例を経験し，その剖検を行ない得たので，その臨床学的所見および病理学的所見に若干の文献的考察を加えて報告する。

　An autopsy case of malignant fibrous histiocytoma in the maxillary sinus is reported.

　Only seven similar cases have been reported in the literature.

　A patient, thirty-nine-year-old Japanese male, was found to have a fibrous tumor, histopathologically confirmed as malignant fibrous histiocytoma, in the maxillary and ethmoidal sinuses in Jan., 1976.

　Radiation therapy and chemotherapy were performed, but the patient developed progressive respiratory failure and died in October 1976.

　Autopsy showed recurrence of the tumor in the left maxillary sinus and metastases into the lungs, pleurae, pancreas, stomach, kidneys, vertebrae and ribs.
