 A new one-stage laryngoplasty was carried out in 4 cases after total laryngectomy. This new surgical technique was performed without any difficulty by using two cervical skin flaps, the thyropharyngealmuscle and the pharyngeal mucous membrane. The tracheopharyngeal fistula was created by one cervical flap, and the other cervical flap became the advancement flap. The pharyngeal mucous membrane was drawn up through the thyropharyngeal muscle and sutured to it, and the pharyngeal fistula with a small opening was created. After laryngoplasty the tracheopharyngeal fistula was created completely and the contraction of the thyropharyngeal muscle prevented the flow of saliva and food into the trachea except in one case with a slight salivary flow. The pharyngeal fistula became the sound sourse, and the postoperative speech articulation and its loudness were better than those obtained in any other methods.
