眼症状を呈した後部副鼻腔炎3症例

POSTERIOR PARANASAL SINUSITIS WITH VISUAL DISTURBANCE,A REPORT OF THREE CASES

　I．緒言

　後部副鼻腔の疾患は，その存在の解剖学的位置から，鼻所見が少なく，むしろ眼症状または頭痛を主訴とする事が多く，X線的に頭蓋底の特異な破壊所見がみられる場合には，脳腫瘍特に脳下垂体腫瘍と誤診されることがある。最近，われわれは高度の視力障害のため，脳腫瘍の疑診のもとに，脳外科にて開頭術をうけた蝶形骨洞mucoceleの1症例に遭遇し，また同様に視力障害を来たした原発性後部篩骨洞のmucoceleの1例，さらに視野狭窄，眼痛など，多彩な愁訴のあつた蝶形骨洞炎の1例を経験した。

　蝶形骨洞mucoceleは比較的稀といわれているが，概して副鼻腔手術後に起こり，失明などの重篤な症状を来たす事がある。したがつてわれわれ耳鼻科医は，副鼻腔手術の既往歴をもつ患者および副鼻腔炎の患者で，特に視力障害を訴える患者には積極的に手術療法を行なうべき事を痛感したので，ここに報告するとともに，後部副鼻腔疾患と眼症状の問題などについても若干の考按を試みた。

　Three cases of posterior paranasal sinusitis that caused visual disturbances are reported.

　The case 1 is a man, aged 29, complaining of bilateral visual disturbances and a frontal headache. The patient was operated with craniotomy for a suspected meningioma. But, a phenoidal mucocele that involved the ethmoid sinus was revealed in the procedure. Sphenoidectomy was performed without a remarkable recovery of the vision.

　In the second case, a man, aged 60, complained of right visual disturbance and a dull headache. The patient was operated for primary posterior ethmoidal sinus mucocele with a complete recovery.

　The case 3 is a man, aged 42, complaining of narrowing of visual field and ophthalmoplegia. The patient made a full recovery by establishing a drainage of the sphenoidal sinus.
