Histiocytosis Xの耳鼻咽喉科所見—2例報告と本邦文献の統計的観察

CLINICAL OBSERVATlONS OF HISTlOCYTOSIS X, A REPORT OF TWO CASES

　I．はじめに

　耳鼻咽喉科領域のHistiocytosis Xは，非常にまれな疾患で，教科書にもHistiocytosis Xの項はまつたくないのが大方で，散見される本邦の報告でも，依然，Hand-Schüller-Christian病（以下H. S. C. と略す），Letterer-Siwe病（以下L. S. とす），または骨好酸球肉芽腫（以下E. G. とす）と報告されているものが多い。Histiocytosis X，特に前二者は，小児に発症する全身性疾患であるために小児科を訪れることが多い。しかし，豊富な耳鼻咽喉科所見をもつことが多く，耳鼻科医としては念頭に置いておかないと誤診をしたり，治療法を誤つたり，あたら貴重な症例を看過してしまう。

　Histiocytosis Xは，Eosinophilic granuloma，Hand-Schüller-Christian病およびLetterer-Siwe病の三者を一括した病理学的名称で，これは三者の病理組織像が非常に類似し，病理像から三者を鑑別することが不可能なことが多いところから，1953年Lichtenstein^7）が，Histiocytosis Xと命名したことに始まる。しかし，定型的症例では，臨床的に三者ははつきり異なつた病態を示す。したがつて，Lichtenstein^7）は，E. G. をchronic Histiocytosis X，H. S. C. をchronic disseminated Histiocytosis X，そしてL. S. をacute disseminatedHistiocytosis Xと細別している。また，Oberman^5）は，HistiocytosisをⅠ，Ⅱ，Ⅲ群に分けて臨床的考察を行なつている。

　Two cases of histiocytosis X are reported. The first case was a male infant, aged 2, who had mastoidectomy, elsewhere, which failed to heal forming a chronic fistula. The second case was a girl, aged 6, with chronic persistent otorhea. The clinical findings in both cases are described in detail.

　The diagnosis of histiocytosis X in both cases was made by microscopic examination. Both cases are improving under the regimen of steroid therapy.

　The authors maintain that histiocytosis X should be clinically classified into eosinophilic granuloma, Hand-Schaller-Christian disease and Letterer-Siwe disease.

　There are 50 cases of Hand-Schaller-Christian disease reported in the literature of this country; 88% of cases occur in the age-group 1-5, while the sex difference is 3:2 in favor of the male.
