Irradiation failure for any intraoral malignancies should be followed by aggressive surgical procedure which may leave an extensive postoperative defect.

Direct suturing of the defect in the mouth is technically feasible in most cases. However, it is apt to meet disadvantages in recovery of oral functions. Patients most likely complain difficulty of phonation and mastication of foods as the result of fixation of the tongue anchored to the buccal mucosa.

The mucosal defect may be reconstructed by a pedicled skin flap taken from the forehead which will also regain the functions of the oral cavity and the mobility of the tongue.

Although the present technic is not cosmetically perfect, it would enable the patient to phonate and masticate quite well postoperatively.
