低体温麻酔下に摘出した頸部気管adenoid cystic carcinomaの1例

REMOVAL OF TRACHEAL MALIGNANCY UNDER HYPOTHERMIA, A CASE REPORT

　Ⅰ．緒言

　気管に発生する円柱腫様腺腫（cylindromatousadenomaまたはadenoid cystic carcinoma）については，本邦においても近年若干の報告例を見るが，その腫瘍学上の特異性とともに，発生部位の特殊な状況も関連して臨床上問題点の少なくない疾患の1つということができよう。

　最近私どもは頸部気管に発生した同腫瘍に対して，低体温麻酔下に，経気管的に摘出する経験を持つた。この腫瘍の一般的な問題点についてはすでに多くの人がこれに触れ，また気管に発生した同腫瘍については最近石井ら^1）が詳細な総説を発表しているので，本論文ではその治療手技について主として報告し，また考察を加えてみたいと思う。

　A woman, aged 35, complained of a severe inspiratory dyspnea. By means of x-ray and endoscopic examinations it was found that the patient had developed a large tracheal tumor attached in the frontal region of the neck.

　Histological study of the biopsied specimen showed the growth to be a cylindromatous adenoma.

　Under hypothermia the tumor was approached through a longitudinal incision of the cervical trachea for a complete removal.

　The authors conclude that hypothermia is the most practical method of anaesthesia which is an efficient means of avoiding excessive hemorrhage as well as a state of anoxia, especially the latter which is highly expected in the manipulation of the trachea of this type.
