涙嚢鼻腔吻合術3例の経験

SOME EXPERIENCE IN DACRYOCYTORHINOSTOMY, A REPORT OF THREE CASES

　Ⅰ．緒言

　流涙を主徴とする慢性涙嚢炎は，鼻涙管の狭窄，閉塞による二次感染によつて発生し，その原因は，トラコーマなどの眼科的疾患および鼻副鼻腔疾患が考えられている。この涙嚢炎の手術的療法として，眼科的立場から主として涙嚢摘出術が，鼻科的立場からいわゆる鼻涙管開放術が行なわれているが，現在のところ耳鼻科医にとつて，鼻涙管開放術は身近に行なわれている手術ではない。近年眼科的および耳鼻科的な成因に加えて，外傷に起因する涙道狭窄，閉塞の報告に接することが多く，交通外傷の激増，頻発している今日，耳鼻科医は，涙道狭窄，閉塞の症例に今までより頻繁に遭遇し，対処せねばならないことが多くなると考えられる。最近われわれは，副鼻腔根治手術後に慢性涙嚢炎を起こした2例および外傷のため鼻根部を強打し，鼻涙管の狭窄をきたし涙嚢炎を併発した1例に涙嚢鼻腔吻合術を施行し好結果を得ることができたので報告する。

　There are two important causes that may lead to the formation of dacryocystitis are that which arise as a complication following sinus operation and the other is due to traumatic accidents.

　The authors performed dacryocytorhinostomy in treatment of chronic dacryocystitis in 3 cases in whom two were caused as an complication of sinus operation while the other one was due to external trauma.

　The author's experience in these procedures are described and commented in detail.
