聴神経腫瘍を疑わせた側頭骨転移癌の1例

METASTATIC CANCER OF THE TEMPORAL BONE THAT SIMULATED AN EIGHTH NERVE TUMOR

　Ⅰ．緒言

　側頭骨への癌転移は，比較的稀である。就中，内耳道領域に転移を生じ，小脳橋角部症状を呈した症例の報告はきわめて少ない。

　最近われわれは，耳症状と顔面神経麻痺をもつてはじまり，聴神経腫瘍が疑われた側頭骨転移癌の1例を経験したので，その診断経過ならびに剖検所見を報告する。

　A man, aged 34, complained of the deafness of the right ear with facial paralysis of the same side.

　The examination of the patient revealed a total deafness of the right ear, complete semilateral facial paralysis, gaze nystagmus, right abducens paralysis, bilateral papilledema and increase of protein in the cerebrospinal fluid.

　An acoustic neurinoma was suspected and the patient was operated.

　The operation revealed a strawberry colored growth the size of a cherry arising from the periostium of the petrous portion of the temporal bone. Biopsy specimens were taken

　The pathology report revealed a metastatic carcinoma (adenoid cystic carcinoma).
