鼻アレルギー診断用誘発ディスクの臨床的検討

NASAL PROVOCATION TEST IN NASAL ALLERGY BY PAPER DISC METHOD

　鼻アレルギーの診断特に抗原検索に鼻誘発反応が不可欠であるのは多くの人により認められている^1）。鼻アレルギーの診断ばかりではなく，気管支の誘発試験の操作が複雑で副作用を伴うため，気管支喘息の診断に鼻粘膜誘発で代用できるとも報告されている^2）。

　さて鼻粘膜誘発試験は多くの人により試みられているが，その方法，判定基準など必ずしも一定せず，それゆえ報告者の成績にくいちがいがあつた。われわれは今回濾紙に抗原抽出液をしみこませ，乾燥したもの（誘発ディスクと仮称）を用いて，誘発試験を行ない，臨床的応用の価値を検討した。

　The nasal provocation test with the use of dried paper disc which contained house dust or ragweed allergen extract was carried out in non-allergic and nasal allergy subjects. Approximately 70% of the patients with positive skin test with offending allergen extract was provocated by the paper disc of corresponding extract, and 30% of the patients with negative skin test was also provocated. Of those with negative skin test but positive provocation test, 5% was presumed to be of false positive provocation due to non specific irritaion of paper and the rest was probably of false negative skin test due to weak concentration of allergen extract. The nasal provocation test by the paper disc method was useful in diagnosis of causal allergen in nasal allergy.
