重篤な鼻出血症例について

A SEVERE EPISTAXIS

　Ⅰ．はじめに

　鼻出血を大別すると，血液疾患，急性熱性伝染病，薬物中毒，ビタミン欠乏，循環器障害など，全身的な原因に伴つて起こる症候性のものと，外傷，炎症，腫瘍，手術など局所的なもの，さらに代償性の場合とに分けられる。

　このなかで問題とされているのが，外見的に局所や全身的に所見の見出されない特発性の鼻出血である。鼻出血は耳鼻咽喉科の診療に従事しているわれわれにとつて，決してまれな疾患ではなく，日常しばしば遭遇するものである。したがつて診療上その占める比重も大きい。ところが，鼻腔の狭小，粘膜の過敏性などの解剖学的根拠や，湧出する血上液，嚏，咳嗽，さらに脳貧血などの患者の状態により，出血部位の確認，完全な止血に難渋する症例が少なくない。われわれは，当科において入院加療した重篤な症例について，調査する機会を得たので報告する。

　An analysis of 5 cases of epistaxis that seemed to be interesting which occurred in the recent 5 years is reported. In good many cases the causative factor was not clear but in a few it had pointed to a recent attack of influenza, excessive drinking or some physical exhaustion. As to the site of the hemorrhage, Kieselbach's area was found in 3 cases while inferior turbinate in 1 and mucous membrane of the nasal cavity in general was the other. Also by general physical conditions pernicious anemia, hemolytic diathesis, essential hypertension and drug allergy were found in some cases.
