重篤な再生不良性貧血に合併した鼻副鼻腔アスペルギルス症2症例

Nasal and Paranasal Aspergillosis in Two Patients with Aplastic Anemia

I．はじめに

　悪性腫瘍およびその治療などによって免疫抑制状態になった患者では弱毒菌や真菌による重篤な感染症にしばしば遭遇する。肺アスペルギルス症に代表されるアスペルギルス症もこのような患者の致死的感染症として注目されており，近年その増加が報告^1〜4）されている。鼻副鼻腔のアスペルギルス症はHoraらによってinvagive typeとnon-invasive typeに分類されている^5）が，McGill^6）は免疫抑制患者に発症し急速に進行する予後不良の鼻副鼻腔アスペルギルス症をさらにfulminanttypeとして分類することを提唱している。今回われわれは再生不良性貧血に合併し急速な壊死性骨粘膜炎を呈しいわゆるfulminant typeに属すると思われる鼻副鼻腔アスペルギルス症2例を経験したので，その臨床経過を提示するとともに診断と治療法につき若干の文献的考察を加えて報告する。

　Nasal and paranasal aspergillosis was encountered in two patients with aplastic anemia. The patients had marked granulocytopenia for at least two months. In one patient who died of sepsis and pneumonia, the diagnosis was established six weeks after the infiltration. In another patient, antifungal therapy was started within nine days, but his bone mallow function was suppressed and no improvement of aspergillosis was recognized at the time of report. The literature on invasive aspergillosis in immunosuppressed patients was reviewed.
