血管性他覚的耳鳴の1例—耳科領域疾患に対する脳血管撮影の診断的意義（2）

OBJECTIVE TINNITUS AURIUM DUE TO VASCULAR DISTURBANCE

　Ⅰ．緒言

　日常われわれが診察する耳鳴は，大多数が自覚的耳鳴であり，他覚的耳鳴の患者を診察する機会は比較的まれである。他覚的耳鳴はいわゆる広義の耳鳴に入るもので，患者自身はもちろん患者以外の人によつても患者と同様の雑音が聴取し得る場合で，その音源を推定または確認できるものである。また他覚的耳鳴はその音源の種類により筋肉性，血管性などに分類される。

　われわれは今回頭部外傷後に血管性他覚的耳鳴を発来した症例について，脳血管撮影によりその音源を確認し，総頸動脈結紮によりこれを治療した経験を得たのでここに報告する。

　Stroboscopic X-ray picture following visualization of the cerebral vessels showed a close anastomosis of these vessels. Because of this anatomic peculiarity the left common artery was ligated for treatment of objective tinnitus. The treatment was a success.

　The author emphasizes the importance of proper visualization of the cerebral vessels in the diagnosis of tinnitus aurium.
