骨格外症候を伴つたFibrous dysplasiaについて

FIBROUS DYSPLASIA WITH EXTRA-SKELETAL SYNDROME

　Ⅰ．はじめに

　Fibrous dysplasiaという名称はLichtenstein ＆ Jaffe^5）（1938）によつて提唱された。従来骨腫瘍の診断および術語の用い方には少なからぬ混乱を招いていた。各報告者がそれぞれの所見を強調するあまりいろいろな名称を与えた感がある当骨疾患も，1942年Lichtenstein ＆ Jaffe^6）が90例について報告して以来一応明確な性格を描き出されるに至つた。最近の欧米文献および著書にはいずれもFibrous dysplasiaと記載されている。本邦においては1942年大井^12）がOsteodysplasia fibrosa（線維性骨異形成症）Lichtensteinなる名称を提唱したが，戦後整形外科の方で邦訳せずFibrous dysplasiaという名称が普及している。しかしわれわれの科では術語の統一がみられない。荒木・平野^14）（1955）は上顎骨及び前頭骨のFibrous dysplasiaの1例として，岡田^21）（1965）はFibro-osseous dysplasiaについてと報告しているが，梅田・豊田^18）（1964）崎川・富田^20）（1965）は邦訳して上顎骨線維性骨異形成症の1例として，柳原^15）（1956），山本^17）（1962），宮内・黒沢^19）（1964）は旧名で上顎骨線維性骨炎の1例として報告している。著者は最近耳鼻科領域で報告まれな骨格外症候（皮膚色素沈着異常および内分泌障害）を伴つたFibrous dysplasiaの1例を経験したので報告する。

　A case of fibrous dysposia is reported. A 19 year old man complained of swelling on the left side of the face involving the base of nasal ala. With radical operation the left maxillary sinus was opened. The sinus cavity was filled with multilocular cysts and the surrrounding structures inner wall of the maxillary cavity, ethmoids, the hard palate and the posterior end of the inferior turbinate were covered by grayish and reddish fibrous tissues. Microscopic examination of these tissues showed them to be a newly formed bony tissues with irregular inner structures. From the absence of the growth of beard and hair under the armpit alond with the poverty of pudendal hair pituitary dysfunction was suspected in this case.
