電撃性耳性脳膜炎の1例

PRECIPITATED ATTACK OF MENINGITIS FROM AURAL ORIGIN

　Ⅰ．緒言

　中耳炎に起因する頭蓋内合併症は抗生物質の進歩とともに激減し，ことに急性中耳炎に続発する合併症は稀有のものとなつた。

　急性中耳炎罹患後短時日の間に急激に脳膜炎を起すいわゆる電撃性中耳炎（耳性早期脳膜炎）の予後は絶対不良であるとされ，報告例の大部分は死の転機をとつている。抗生物質の進歩した今日においても，発病の電撃性，症状の重篤さ，耐性菌の出現などからしてもつとも怖るべき合併症の一つといわねばなるまい。

　A 57 year old woman with negative history of previous ear disease suddenly developed meningitis as a complication about sixteen hours after the onset of acute otitis media. The patient complained of severe headache and marked tinnitus aurium. The blood sedimentation rate was found to be markedly increased. The mastoid was opened for drainage and removal and it showed break-down in the bony trabeculi. Bacterial identification was unsuccessful but mucosus infection was suspected. The patient recovered under large dose administration of antibiotics. No recurrence of the disease is seen today year after the operation.
