 Sasaki reports that 3 cases of otitic meningitis which were caused by bacillus influenza in spite of having been generally considered as difficulty of cure were nevertheless successfully cured. The age of the patients was all below 1 year. In 2 cases the causative agent was that of bacillus Influenza pure and simple, while the third revealed a mixed infection which composed of bac. influenza, staphylococcus albus and Frankel's diplococcus. In the progress of the disease, the patients Invariably developed otitis media subsequent to the state of common cold; and, within the following 7-10 days symptoms and signs of meningitis were definitely established. Examination of tympanic membrane in these cases failed to reveal signs of any import excepting that it looked rather pale in one, and slightly congested or edematous in others; no hemorrhagic spots were visible and, as a whole, the picture presented was that of serous otitis. However, compared to this picture of the tympanic membrane the destruction of mastoid cells in each case was out of all proportions. Spinal fluid examination also revealed numerous cell contents., yet, in general, manifestations of symptoms of meningitis were relatively slight: The treatment consisted of intrathecal administration of 50-150 mg doses of streptomycin, the total of which amounted to 0.85-1.0 g., and iotramuscular injections of 50-100 mg per dose of streptomycin, which amounted to 1-2 grams in total. Treatment of the case with mixed infection required 2 grams of streptomycin intramuscularly in addition to 1,000,0000 units of penicillin and 15 g. of homosulfamin. The author believes that successful recovery of the 3 cases has been efected by early performance of mastoidectomy in each.
