With 26 cases of radical cervical resection which were performed during the period of recent 3 years. statistical observations as well as the method of care of the cases in which the common and or internal carotid were either ligated or severed. Mention isalso made on cases in whom where it was not possible to execute bilateral ligation of the common carotid.

With the following conditions, there appe-ared to be, in general, a less number of posto-perative hemilateral paralysis: When the pa-tient was above the age of 60; when the amount of blood pressure changes were less at the time of the arterial ligation; when the amount of preoperative irradiation was grea-ter in amount; and when peri-vascular scartissue formation was greater in amount.

The postoperative complications were al-most unavoidable when ligating procedures were performed as an one-step operation, but, with two-step method there was no co-mplication with the ligation of the common carotid alone. However, with the ligation of the internal carotid that followed the preced-ing procedure there were hemilateral paraly-sis in approximately 50% of cases.
