 In order to avoid the troublesome inflammatory product of crusting in the concave area of the external auditory canal following the usual method of tympanoplasty, the authors devised a new method which they called meatotympanoplasty. In this procedure the skin of the external canal is separated from the underlying bony canal in a tubular state and the bony canal itself is also preserved intact. The mastoid antrum is adequately widened from the posterior aspect. The tympanic membrane repair is accomplished by means of connective tissue graft. Becausethere is a minimum disturbance to the external canal the troublesome postoperative crusting is avoided and very little attention is necessary in the aftercare. Statistically, postoperative hearidg gain was not significantly different from that of the ordinary tympanoplasty. However, from clinical experiments and the experiments using plaster model and dissected human temporal bone it was demonstrated a large posterior fossa in the external canal necessarily entailed a dip of 5~15 db in 2000~8000 cps.
